Comes from the late Mundane Recovery or so. There used to be a great fog cutting off a couple of main landmasses. This was a minorly cursed fog at a time when humans were regaining medieval-ish technology and magic was uncommon and not understood. They eventually learned to navigate the fog but didn't fully understand what they were doing. After all their calculations came together ships would always be off course by a predictable but unexplained amount. This predictable value became known as the Fog Error (or similar that eventually morphed to that).

I'm not really sure where to go from here. It could be like 'they have a fog of error about them' or 'they're in a fog of error' but I don't know what that actually means. Maybe like describing something as being fog to mean you don't really know why it is working it just is? I don't know
